Rehabilitation

Our staff wants you and your family to play an active role in setting your recovery goals and will help you achieve them. Under the supervision of your physician, our highly qualified therapists will work closely with you to aid in your recovery.

Horizons wants our residents to achieve functional independence in their daily lives. Our team of skilled professionals will provide rehabilitative and restorative care for you or your loved one recovering from strokes, injuries, illness or surgery.

Our therapy staff includes physical, occupational and speech therapists that are able to provide rehabilitation services seven days a week.

Our physical, speech, and occupational therapists participate in planning your return home. Working with other members of the healthcare team, we will help you and your family continue the care at home. We will help you locate home based services and local support groups in order to assist you in returning to the community.
